Symptoms of adrenal fatigue: what people list and what else can cause them

The symptoms of adrenal fatigue, as the idea is usually described, are tiredness, poor sleep, trouble coping with stress, body aches, digestive problems and a reliance on caffeine. Adrenal fatigue is not a recognized medical diagnosis, so no set of symptoms identifies it. Each item on the list is also a feature of conditions that are recognized.

This page goes through the list, what else can sit behind each symptom, and the signs that point to real adrenal conditions. For the evidence on the idea itself, see is adrenal fatigue real.

The symptom list and its other names

The 2024 paper describes adrenal fatigue as an alleged condition, proposed to follow chronic stress, and characterized by nonspecific symptoms: fatigue, sleep disturbances, difficulty coping, body aches, digestive problems and dependency on caffeine. Online lists are longer and vary from site to site. The names differ too, including adrenal burnout, adrenal exhaustion and adrenal gland fatigue, but they point at the same claim.

The 2016 systematic review looked at 58 studies that tested cortisol or adrenal function in relation to fatigue, exhaustion or burnout, 33 in healthy people and 25 in people with symptoms. It found conflicting results and no substantiation for the condition. That is why a symptom checklist can’t give you a diagnosis. Symptoms that fit almost anyone at some point will fit you too.

What else can cause each symptom

Symptom on the listConditions and causes to ask about
Tiredness that rest doesn’t fixMedlinePlus lists diabetes, heart, lung, liver, kidney and thyroid disease, anemia, infections, sleep apnea and other sleep disorders, depression and anxiety, and some medicines. It also describes ME/CFS, where fatigue lasts a long time and doesn’t improve with rest.
Poor sleepSleep apnea and other sleep disorders, which MedlinePlus lists as causes of fatigue. MedlinePlus also says too much caffeine can keep you from a good night’s sleep. Trouble sleeping is a symptom of an overactive thyroid.
Trouble coping, low mood, irritabilityDepression and anxiety, which MedlinePlus lists as causes of fatigue. Depression goes with an underactive thyroid, nervousness and irritability with an overactive one, and irritability and depression with adrenal insufficiency.
Body achesUntreated pain and fibromyalgia (MedlinePlus). Joint and muscle pain is a symptom of an underactive thyroid. Joint pain appears on NIDDK’s list for adrenal insufficiency.
Digestive problemsConstipation with an underactive thyroid, frequent bowel movements or diarrhea with an overactive one, and nausea, vomiting, diarrhea or belly pain with adrenal insufficiency.
Needing caffeine to functionCaffeine use isn’t a sign of any one condition. The causes of fatigue above are the place to look. MedlinePlus lists not getting enough sleep and staying up late among them.
Weakness and dizzinessLow iron. MedlinePlus lists weakness, fatigue and dizziness among its symptoms. Also adrenal insufficiency, where NIDDK lists dizziness on standing.

None of this says that one of these is your cause. It shows how many things look the same. Our page on adrenal or thyroid tests for tiredness explains which blood tests help sort them out.

Symptoms in females

Many searches add “in females.” The 2024 paper’s description of adrenal fatigue doesn’t include any symptom specific to women. What the sources do show is that some conditions behind these symptoms differ by sex:

  • NIDDK says women are much more likely than men to develop hypothyroidism, and heavy or irregular periods are on its symptom list.
  • MedlinePlus lists heavy menstrual bleeding and pregnancy among the situations that raise the risk of low iron.
  • Pregnancy is also on MedlinePlus’s list of causes of fatigue.
  • Irregular or absent periods and loss of interest in sex are on NIDDK’s list for adrenal insufficiency.

If periods have changed along with tiredness, mention it to a clinician. It is useful information whatever the cause.

Symptoms that point to real adrenal conditions

These are different from the adrenal fatigue list. NIDDK describes the symptoms of adrenal insufficiency (too little cortisol) as:

  • Long-lasting fatigue, muscle weakness, loss of appetite and weight loss.
  • Belly pain, nausea, vomiting or diarrhea.
  • Low blood pressure that drops further on standing, with dizziness or fainting.
  • Craving salty foods, and low blood sugar.
  • In Addison’s disease, darkening of the skin, most visible on scars, skin folds, pressure points, lips and the lining of the cheek.

MedlinePlus lists the symptoms of too much cortisol (Cushing syndrome) as weight gain, thin arms and legs, a round face, extra fat at the base of the neck or between the shoulder blades, easy bruising, wide purple streaks on the stomach, breasts, hips and under the arms, and muscle weakness.

Having one of these doesn’t mean you have either condition. NIDDK notes that adrenal insufficiency symptoms come on slowly and can be mistaken for other illnesses. In a hospital series from Poland of 60 people with Addison’s disease, the average time from first symptoms to diagnosis was about 6 months without a crisis and about 9 with a crisis or an imminent one (Int J Endocrinol, 2013). That supports taking persistent unexplained symptoms to a clinician early. See low cortisol symptoms and high cortisol symptoms.

Call 911 or go to the nearest emergency room for vomiting you can’t stop together with severe weakness, or for fainting. See adrenal crisis.

What about the 7 stages of adrenal fatigue?

You will see “stages” of adrenal fatigue online. None of the medical sources used on this page describe stages of adrenal fatigue. The 2016 review found no proof the condition exists, so there is nothing to stage. A stage quiz is not a validated test either: the 2024 paper says online symptom scoring systems have never been validated.

When to see a clinician

MedlinePlus says to contact your provider if you have been tired for weeks. It says they will ask about your sleep, daily activities, appetite and exercise, do a physical exam and likely order lab tests. Go sooner for the signs in the section above.

Two more cautions from the 2024 paper:

  • Some websites sell “adrenal extracts” made from cattle adrenal glands that contain real steroid hormones. The paper warns these can cause adrenal insufficiency.
  • If you take a steroid medicine, don’t stop or change it because of anything you read. Ask the prescriber.

Questions people ask

Is “adrenal fatigue disease” a real disease?

No. It is not a recognized diagnosis. Adrenal insufficiency and Cushing syndrome are real conditions, and they have defined tests.

Can a cortisol test show adrenal fatigue symptoms?

No. The 2024 paper says the salivary cortisol profiles sold for adrenal fatigue haven’t been validated. A cortisol test is used when a real cortisol condition is suspected, and even then a single result can’t settle it. See the cortisol test guide.

Are the symptoms the same in men and women?

The 2024 paper’s description of adrenal fatigue doesn’t separate men and women. Some other causes of the same symptoms do differ by sex: NIDDK says hypothyroidism is much more common in women, and MedlinePlus lists heavy menstrual bleeding as a risk for low iron.
